BCWBS Wheelchair Loan Program

Please fill out the following application for the BCWBS wheelchair loan program. In order to be eligible to apply for a BCWBS sport wheelchair you must be an active participant in a BC Wheelchair Basketball Society program; be a resident of BC; be a member in good standing with BCWBS; and show a substantial need for a chair.
